Case Manager

Lighthouse Youth & Family Services has a Case Manager position available in Community Juvenile Justice. As part of a mission focused, innovative, diversion program, the successful Ross County Juvenile Justice Case Manager will work with adolescents who are being diverted from juvenile justice residential treatment placements. The Case Manager will work closely with other professionals in providing treatment services in the areas of mental health, employment, education, and criminal offending behaviors.
Monday – Friday, 10am to 6:00 pm.
Essential Functions:
- Assess client needs on an ongoing basis and determine the appropriate level of care and services needed.
- Provide support and advocacy to clients and families through case management related activities.
- Assist client with identification of strategies or treatment options. Participate in and utilize strengths-based planning.
- Uses specialized knowledge of chemical dependency and addiction theory, CBT curriculum and trauma informed care concepts to assist youth and families to improve their capacity to abstain from substance abuse through assessment, interventions, and advocacy.
- Establish and maintain professional relationships with youth, family and service providing partners.
- Coordinate and assist in crisis management and stabilization as needed.
- Implement & coordinate appropriate individualized service plans, reports, discharge and case closure plans in conjunction with treatment support team.
- Document and submit required billable service units, meeting utilization standards as set by service line, program and/or agency.
- Initiate services within the expected time as set by agency standards.
- Complete progress notes and service documentation within specified timeframe per Lighthouse clinical documentation standards.
- Maintain files in accordance with all regulatory compliance standards.
- Act as liaison between referral sources, external partners and internal LYS service lines and programs.
- Participate in appropriate professional development activities and complete required training.
- Collaborate with all members of the clinical team to ensure quality, continuity, and care for clients.
Qualifications:
- Bachelor’s degree in Human Services or closely related field or HS/Associates with three years of relevant experience.
- Car insurance and safe driving record
- Excellent interpersonal and written communication skills
- Demonstrated computer proficiency.
