Lighthouse Youth & Family Services has a full-time Social Worker position available in Foster Care & Adoption.
The primary schedule for this role is Monday through Friday from 8-430pm (flexible schedule available) with limited remote work available.
Competitive pay based on licensure/education
Essential Job Functions:
- Assess client and family needs and determine services needed.
- Act as liaison between referral sources, external partners and internal LYS service lines & programs.
- Provide support and advocacy to clients and families through appropriate therapeutic interventions.
- Provide case management services for clients which includes linking to referral resources for ongoing support.
- Develop and implement effective individualized service plans for clients.
- Complete documentation and maintain files in accordance with all regulatory compliance standards.
Qualifications:
- Dependent Licensure, LSW or PC, required.
- Excellent interpersonal and written communication skills, required.
- Demonstrated computer proficiency and basic office equipment, required.
- Knowledge and/or experience with Trauma Informed Care, preferred.
