Home Health/Community Health Manager - Full Time Days

We have a full-time position (40hours per week) in our Home Health/Community Health Department Manager (RN required). Our Home Health/Community Health department manages an average census of 60-70 patients and serves as the community health and public health of Plymouth County, Iowa.

Responsibilities:
- Act as the liaison between the hospital’s Administration, the Board of Trustees, the staff and the community.
- Responsible for the care provided to our home health patients and for the affairs of the Community Health Department.
- Provide leadership to skilled services in Medicare Certified Home Health agency including nursing, home care aides, and therapy services.
- Ensuring compliance with Medicare Conditions of Participation.
- Grant management of public health services for Plymouth County.
- Continue evaluation of community health needs and development of health improvement plans.
- Participate in community and professional organizations to enhance home health and public health programs.
Requirements:
- Previous management experience is a plus.
- Licensed as a registered nurse (RN), in Iowa and/or meets the requirements of multi-state licensure required.
- Valid driver’s license and proof of insurance.
