What are the responsibilities and job description for the Hospice Field RN Case Manager position at Lightways Hospice and Serious Illness Care?
Founded in 1982 as Joliet Area Community Hospice, Lightways Hospice and Serious Illness is an independent, non-profit healthcare provider licensed in 11 counties in Illinois. We have a state-of-the-art facility and the first free standing hospice inpatient unit in Illinois. We have a 35-year history for providing compassionate professional care to terminally ill patients and their families. We are state-licensed and Medicare/Medicaid certified.
We are currently seeking a full-time RN to care for our patients residing in their private homes, nursing homes, or long-term care facilities. This position will primarily cover Northern Cook and Lake counties. Our RN Case Managers have a caseload of patients they are responsible for managing according to the plan of care including assistance with activities of daily living. May be asked to assist with referrals/admissions based on caseload. This position works Mon-Fri and must have the availability to do minimum of 20 hours of on-call per month, may increase based on staffing.
The qualified candidate will have a minimum of one (1) year nursing experience in home health, critical care or long-term care, although hospice experience is preferred.
