The ACT Specialist provides community based support, therapy and advocacy to mentally ill consumers and their families. This professional collaborates with psychiatric and allied team members in assessment, diagnosing, treatment planning, and intervention.

Essential Duties AND Responsibilities include, but may not be limited to the following :

Provides ACT services / case management, linkage, and advocacy.

Reports to work on time as scheduled and at the location assigned.

Completes all required trainings.

Responsible for transporting consumers to community appointments, clinic appointments, activities and other resources as identified in their person-centered plan.

Assists clients in acquiring safe and stable housing.

Assists clients in completing applications for appropriate entitlements.

Assesses personal needs of ACT clients in areas such as living situation, financial, self-care, vocations, and symptom identification and care, and develops a person-centered treatment plan.

Develops meaningful and comprehensive assessments.

Develops relevant and measurable treatment goals and objectives based on assessment information.

Facilitates client involvement in the treatment planning process.

Practices Person Centered Planning principles.

Works as part of a treatment team to facilitate efficient operation of the program and client services.

Participates in all team meetings, treatment planning and plan of service meetings.

Participates in developing problem solving strategies as related to client and program needs.

Provides conscientious and reliable delivery of clinical services.

Demonstrates knowledge of diverse and relevant therapeutic interventions.

Conducts individual and / or group therapy as assigned.

Utilizes appropriate and effective interventions based on client needs.

Creates a supportive environment conducive to client growth.

Provides crisis intervention.

Participates in monthly 24-hour crisis coverage or as assigned.

Provides support and referral to needed services during crises.

Completes all documentation in a timely fashion, and meets requirements regarding caseload, direct service time, and completes billing, progress notes, assessments, treatment plans, and treatment plan reviews within required timeframes.

Maintains minimum direct service hours of 80 per month.

When appropriate, completes other assigned responsibilities in a reasonable established timeframe, within quality expectations.

Education AND Experience

Masters degree in social work, counseling or related fields from an accredited college or university required.

Preferred 2 years experience in working with the mentally ill population.

CREDENTIALING / LICENSURE . State of Michigan (LARA) LLMSW, LMSW, LLC, LPC licensure required. Employee is responsible for obtaining the required supervision for their respective licensure.

The therapist comprehensively assesses client needs on multi-dimensional domains (behaviors, health, education, financial security, relationships, meaningful activities, quality of life, etc.). Based on client and family needs, the therapist applies interventions to decrease challenging behaviors and distressed family dynamics, improve communication and quality of life. The therapist will be able to identify what is under the surface and respectfully explore while maintaining unconditional positive regard.

Possession of a valid Michigan Driver’s License with a good driving record

Has maintained a valid Michigan Driver’s License for at least 3 years.
