Job Description:
The ACT Specialist provides community-based support, therapy, and advocacy to individuals with mental illness and their families.
- Transports clients to community appointments, clinic appointments, activities, and other resources as identified in their person-centered plan.
- Assists clients in acquiring safe and stable housing.
- Completes applications for entitlements on behalf of clients.
- Conducts assessments to identify personal needs of ACT clients in areas such as living situation, financial stability, self-care, vocation, and symptom identification and management.
Develops a comprehensive treatment plan based on client needs and works collaboratively with the treatment team to ensure efficient program operation and delivery of client services.
Ensures timely completion of all documentation, including caseload management, direct service time, billing, progress notes, assessments, treatment plans, and treatment plan reviews.
Maintains licensure through the State of Michigan (LARA) LLMSW, LMSW, LLC, LPC, and obtains required supervision as needed.
Utilizes effective written and oral communication skills to interact with internal staff, teams, and the community served.
Manages caseload effectively, consistently meeting required timeframes, and delivers high-quality work while ensuring consumer satisfaction.
Establishes professional working relationships with all staff and articulates the agency's view when networking within the community.
