What are the responsibilities and job description for the Mental Health Advocate position at Lincoln Behavioral Services?
Main Responsibilities
- Provide community-based support and therapy services to mentally ill consumers and their families
- Transport consumers to appointments, activities, and other resources as identified in their person-centered plan
- Assist clients in acquiring safe and stable housing
- Assist clients in completing applications for appropriate entitlements
- Assess personal needs of ACT clients in areas such as living situation, financial, self-care, vocations, and symptom identification and care
- Develop and implement person-centered treatment plans
- Work collaboratively with other healthcare professionals to facilitate efficient operation of the program and client services
