Job Description
Job Title: Case Manager
Reports To: Domestic Violence Operations Director
Employment Status: Salary. Typically 40 hours per week, flexible hours as necessary.
Purpose/Objective: Provide case management and lead support efforts for victims of domestic violence who are seeking services from the Lincoln County Coalition Against Domestic Violence (LCCADV).
Responsibilities
- Orient new clients to program requirements; assess client needs; assist clients in developing safety plans and goals; link clients to resources and services that foster safety and self-sufficiency; document client activities and progress toward goals
- Develop, implement, and assess the effectiveness of individual client service plans
- Organize and maintain accurate and up-to-date documentation, including client files and program reports, using appropriate technology
- Work with program staff to develop and maintain a consistent case management process from client intake to discharge/exit.
- Develop and implement programs within the organization and partner with other human service agencies to maximize positive client outcomes
- Work with program staff to provide valid and reliable client and services-related data
- Teach clients essential living skills, social skills, and healthy coping skills. Utilize available evidence-based training materials
- Support positive working relationships within the LCCADV and between the LCCADV and other partner agencies
- Develop community connections to support organizational goals
- Work with Shelter Manager to facilitate transition from Amy’s House (LCCADV Shelter) to safe rehousing
- Maintain confidentiality, ethical behavior, and safety regulations and procedures. Work with staff and clients to maintain these policies at the organizational level
- Work with Executive Director to develop programs leading to best client outcomes and collect program data for reporting and to assess program effectiveness
- Conduct weekly one-hour support group sessions; implement education and motivational approaches; address issues of domestic violence and co-occurring behaviors; assist clients to find solutions for positive change
