Director of Social Services

Lincoln Park Healthcare Campus is currently seeking a Director of Social Services to Join our Team !

QUALIFICATIONS:

Master’s Degree from an accredited College or University in Social Work. Must obtain a NJ LSW and/or NJ CSW. Minimum of 3 years supervisory experience in Long Term Care or health setting preferred. Advanced knowledge of DOH and Federal Regulations relating to provision and Social Services in a Long Term Care Setting.

POSITION SUMMARY:

• Designs, implements and Supervises the Social Services Team based on the facility’s policies and procedures as regulated by the DOH and Federal laws.
• Provides direct care and supervision to Social Services Staff insuring quality of Care and full compliance.
• Assist with planning and implementing a comprehensive social service program which provides for counseling and other support services for residents and families in the Center.
• Must have prior experience with Admissions and Discharges in Nursing Home environment.
• Identify medical-related social needs of residents, provide appropriate services to meet the individual, as well as collective needs of residents and maintains records relating to the residents’ social work needs and care.
• Work directly with residents and families experiencing personal and environmental difficulties or concerns related to the resident’s physical or emotional condition.
• Promote the preservation of the resident’s physical and mental health and to prevent the occurrence or progression of personal or social problems.
• Maintain written record of the frequency and nature of the social service consultation and services provided. Must be educated and well-seasoned with MDS, PASRRS, Advanced Directives, Care Planning, QAPI, etc.
• Ensure and document that residents and/or families have input into the care planning process.
• Act as a liaison between residents, families, outside agencies, and the Center Administrator and Regional Social Worker to ensure that the resident’s rights are maintained and upheld.
