Qualifications and description for a Social Worker, LSW / CSW:
- NJ Social worker, CSW / LSW License
- Conduct Social Service assesments and documents according to state and federal regulations
- provide Physcosocial assesments and relevant parts of Minimum Data Set (MDS)
- Counseling Residents
- Subacute experience highly preferred
