If yes, how you will contribute to the team:

We are seeking a compassionate and dedicated LPN or RN Admissions Nurse to join our team at Linden Grove. As an Admissions Nurse, you will play a vital role in ensuring the smooth transition of new residents into our facility by providing thorough assessments, detailed health evaluations, and ensuring all necessary documentation is completed.

Conduct pre-admission assessments of potential residents, including health history, medication review, and physical exams
Review and verify medical records, lab results, and other necessary documents for accuracy and completeness
Coordinate with families, physicians, and interdisciplinary teams to facilitate smooth admissions and care planning
Ensure proper documentation is maintained in compliance with federal, state, and facility regulations
Provide education and information to families regarding services offered, admissions processes, and health management
Assist in the development of care plans, ensuring they align with the needs of the new residents
Monitor residents’ condition during the admissions process and ensure appropriate care is provided
Work with the admissions team to streamline and improve the intake process for residents
Serve as a point of contact for residents and families throughout the admission process

Must Have:

RN with current licensure to practice in the state
LVN / LPN with current licensure to practice in the state
Strong communication and organizational skills
Basic knowledge of computer use or willingness to learn

Nice to Have:

Experience in Skilled Nursing Facility (SNF) environment
Nursing experience providing direct care to multiple residents
Behavioral care / memory care experience
Experience with Point Click Care (PCC)
