About the Role – Social Service Worker:
As Social Service Worker, you will be a key advocate for our residents, ensuring that their emotional, social, and personal needs are supported. You will work closely with our interdisciplinary team to develop and implement care plans that enhance residents' quality of life.
Your Responsibilities Will Include:
- Advocacy and Support – Assist residents and families with social, emotional, and family issues.
- Care Planning – Participate in resident care planning, identifying social and emotional needs in alignment with medical assessments.
- Compliance and Documentation – Accurately complete Social Services components of MDS, assessments, care plans, and progress notes to meet all state, federal, and facility guidelines.
- Discharge Planning – Develop and coordinate discharge plans to ensure smooth transitions.
- Community Resource Coordination – Make referrals to community agencies and maintain records of services provided.
- Family and Resident Engagement – Serve as a primary point of contact for residents and their families, providing updates, guidance, and support.
- Resident Advocacy – Assist with room changes, help residents adjust to facility life, and ensure their rights are protected and respected.
- Collaboration – Work closely with facility leadership, interdisciplinary teams, and mental health providers to address resident psychosocial needs.
- Community Education – Help educate the public and residents’ families on aging, residents’ rights, and available services.
What We’re Looking For:
- Bachelor’s degree in social work or a related human services field (e.g., sociology, psychology, rehabilitation counseling, special education).
- At least one (1) year of supervised social work experience in a healthcare setting (required); additional healthcare social work experience preferred.
- Strong verbal and written communication skills.
- High level of professionalism and confidentiality.
- Team player with the ability to work across departments and with all levels of staff.
- Customer service-focused, organized, and able to manage multiple priorities under pressure.
