We are seeking a Resident Care Coordinator to join our team at Linden Ponds by Erickson Senior Living. This role will be responsible for coordinating and providing clinical oversight and care to residents within Assisted Living and Memory Care.
The Resident Care Coordinator will work closely with our assisted living manager to complete resident pre-screening/admission, re-admission, and ongoing assessments. They will ensure that resident's clinical assessments reflect their preferences, goals, and individualized clinical needs.
