Description
The Occupational Therapist is supervised by the Assistant Director of SCHC. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgment/problem solving, decision making, and initiative. The responsibilities include, but are not limited to:
A. Perform Occupational Therapy evaluations. Establish treatment plans and obtain physicians orders.
B. Conduct individual therapy sessions in the patient's home.
C. Prepare initial evaluation, progress, recertification, and discharge reports when appropriate.
D. Assist in coordination of Occupational Therapy program with other health care providers.
E. Counsel individual patient/caregiver in relation to patient's specific problems and needs.
F. Participate in patient-centered conferences when appropriate.
G. Evaluate need for equipment, order and maintain equipment and therapy materials.
H. Teach patients, caregivers and staff transferring techniques and Home Exercise Program (HEP) as needed.
I. Participate in and/or present inservice programs.
J. Prepare accurate documentation of services provided.
K. Drive a car, possess a valid driver’s license and have a car available for use.
L. Follow rules and regulations of the NYS and Federal Health Code in regard to the program. Interpret and follow Medicare and Medicaid guidelines.
M. Perform other related duties as required by their manager.
Qualifications
A. Education and Training – An individual who meets one of the following criteria:
1. registered with the American Occupational Therapy Association; or
2. either a graduate of a program in occupational therapy approved by the Council on Medical Education of the American Medical Association in collaboration with the American Occupational Therapy Association or a graduate of a curriculum in occupational therapy which is recognized by the World Federation of Occupational Therapists and is eligible for registration with the American Occupational Therapy Association.
Must be registered and licensed by the New York State Education Department, or have a valid limited permit.
B. Experience - Two years experience preferably with exposure to acute care, rehabilitation or homecare.
C. Character – The individual must possess good moral character, patience and compassion for the chronically ill and disabled.
Salary : $36 - $48
