Description
The Physical Therapist is supervised by the Rehabilitation Manager. The job duties and responsibilities are to be performed while observing the following standards: job standard (knowledge, ability, skills), technical skill, quality, quantity, judgment/problem solving, decision making, and initiative. The responsibilities include, but are not limited to:
A. Perform Physical Therapy evaluations. Establish appropriate plans of care based upon noted impairments.
B. Complete all paperwork in an accurate and timely manner including, but not limited to, evaluations, progress notes, re-evaluations, discharge summaries and screenings.
C. Supervise PTAs, PT/PTA students (when appropriate), and therapy aide.
D. Works as a member of the interdisciplinary team including occupational therapy, speech therapy, nursing, social services, and dietary.
E. Provide education to staff, residents, and/or family members.
F. Participate in meeting with families, residents, and/or interdisciplinary team, as appropriate.
G. Evaluate need for equipment, order and maintain equipment and therapy materials.
H. Participate in and/or present inservice programs.
I. Possess a valid driver’s license and have a car available for use.
J. Have knowledge of, and follow rules and regulations of the NYS and Federal Health Code in regard to the program. Interpret and follow Medicare and Medicaid guidelines.
K. Communicate with the physician and/or interdisciplinary team regarding resident status.
L. Maintain current, accurate clinical records. Documentation is completed in a timely and efficient manner.
M. Participate in scheduling treatment and training sessions.
N. Conduct home evaluations as needed, including coordination with family and resident to ensure resident participation.
O. Complete billing documentation accurately and timely.
P. Provide direction to PTA/CNA and students for completion of plans of care.
Q. Review and cosign PTA and student documentation.
R. Evaluate results of program by observing, noting and evaluating resident’s progress, recommending and implementing adjustments and modifications.
S. Maintain resident confidentiality by following HIPAA guidelines.
T. Follows standard precautions and any further infection control precautions.
U. Maintain state licensure and continuing education requirements.
V. Assist in discharge planning in conjunction with members of the interdisciplinary team. Order necessary equipment for discharge in a timely manner.
W. Able to adjust to multiple interruptions and re-prioritize.
X. Performs other related duties as required by their manager.

Salary: $36.00 - $48 per hour
Qualifications
Qualifications Include:
A. Education and Training - Graduate of a school approved by the American Physical Therapy Association with a bachelor's degree. Must be registered and licensed by the New York State Education Department or have a valid limited permit.
B. Experience - Two years experience preferably with exposure to acute care, rehabilitation or homecare.
C. Character – The individual must possess good moral character, patience and compassion for the chronically ill and disabled.
