Role Summary
Our client is seeking Social Workers for creating outcomes for the patient and family by managing complex psychological and economic co-morbidities. You will provide patients and families education and consultation regarding resources, collaborate to develop a discharge plan and facilitate and monitor its implementation.
Duties and Responsibilities
- Assess patient's and family's psychological risk factors though evaluation.
- Document in medical records patients identified as high risk for readmission.
- Provide intervention in cases involving child abuse / neglect, domestic violence, elder abuse, institutional abuse and sexual assault.
