What are the responsibilities and job description for the Service Coordinator/Case Manager position at LinkAbility Inc?

Service Coordinator (SC): Seeking applicants for the position of Service Coordinator for persons with developmental disabilities. Must have a bachelor's degree from an accredited college or university and experience in the field. Responsible for working with the planning team to develop a person-centered plan and monitoring service(s) received. Each Service Coordinator has a caseload of roughly 35 individuals. Caseloads are set up to see different individuals each month, so it's not all at once. Caseloads are of all ages and consist of varying needs as well.
Service Coordinators are required to log a minimum of 120 hours of service hours per month. As a Targeted Case Management agency, we are contracted with the Department of Mental Health, and we make a promise in our contract for each SC to bill the 120 service hours to ensure our individuals are cared for. During the first two months of employment, we do lessen the billing requirement to give each new Service Coordinator some wiggle room to learn and grow. We are paid for those service hours by Medicaid monthly.
This is a professional career position. Service Coordination is an autonomous-style job that requires someone to be self-motivated. A qualified candidate must have good computer knowledge and professional writing skills. Training, competitive wage, and great benefits are provided. The counties we cover are Taney, Stone, and Ozark with mileage paid. Starting salary is $40K and after the successful completion of your orientation, you will earn a raise and the option to work four 10-hour workdays! We offer bonuses, monthly incentives, and annual raises. We pay 100% of the employee's health benefits and 50% for children.
