Care Coordinator
Duties:
- Schedule and coordinate patient appointments, ensuring efficient use of provider's time
- Maintain accurate and up-to-date patient records
- Communicate with patients regarding appointment reminders, follow-up care, and test results
- Assist with insurance verification and pre-authorization processes
- Collaborate with healthcare providers to ensure seamless patient care coordination
- Provide administrative support to the healthcare team as needed
- 50/50 work from home and out in the field
Experience:
- Social work
- Familiarity with medical terminology and procedures
- Proficient in using electronic medical record (EMR) systems
- Strong organizational skills and attention to detail
- Excellent communication and interpersonal skills
- Ability to multitask and prioritize tasks effectively
Compulsory documentation experience working with Health Homes program and Molina Healthcare's Clinical Care Advance platform
