Key Responsibilities:
- Provide comprehensive nursing care to clients in their homes or community settings.
- Apply evidence-based practices to deliver individualized client care and set goals across six domains: personal health, environmental health, life course development, maternal health, family and friends, and health and human services.
- Participate in quality improvement initiatives and use data to guide and improve practice.
For this role, we are looking for a highly skilled and compassionate nurse who can establish therapeutic relationships with clients and navigate complex healthcare systems.
Requirements:
- A valid license to practice as a Registered Nurse in the State of Oregon.
- At least 2 years of experience as a Nurse Home Visitor or completion of training for the Nurse Family Partnership program.
- Ability to communicate effectively with clients and colleagues.
