Salary Range : $85,500 - $128,000 Signing Bonus & Partial Relocation Package

About the Role
A dedicated and experienced Care Management Services Director is needed to lead and oversee the programmatic development, planning, organization, and management of patient care facilitation. This role is responsible for ensuring the seamless movement of patients through the hospital continuum by managing Case Management, Social Work, Utilization Management, and post-acute service coordination .
This leadership position will be instrumental in refining and advancing the Hospital Case Management program to enhance staff roles and optimize the progression of patient care .
Key Responsibilities :
Lead and manage Case Management, Social Work, Utilization Management, and post-acute services coordination to ensure smooth patient transitions.
Develop and implement hospital-wide care management programs to improve patient flow and outcomes.
Drive process improvements for optimal care coordination and resource utilization.
Work collaboratively with healthcare teams, physicians, and hospital leadership to streamline care facilitation.
Maintain compliance with healthcare regulations, policies, and industry best practices .
Utilize data-driven decision-making to improve patient experiences and operational efficiency.
Must-Have Qualifications :
Current state RN License (Required)
Minimum 5 years of case management experience in a healthcare setting
Proven leadership, communication, and customer relations skills
Preferred Qualifications :
Master’s degree preferred
3-5 years of supervisory experience
Formal case management training preferred
Interview Process :
Step 1 : Initial screening by the Executive Director
Step 2 : Virtual interview with the Director (if criteria are met)
Step 3 : On-site interview for selected candidates
Step 4 : Final offer after all interviews are completed
