Summary
The service coordinator works one to one with Literacy Pittsburgh students to understand their nonacademic barriers to learning, facilitate the development of a plan to reduce those barriers, and coordinate services to overcome barriers. The service coordinator is a resourceful connector, a strong administrator, and a trustworthy ally to Literacy Pittsburgh students.
Essential Functions
Meets with students individually to assess non-academic barriers to learning including education proficiency level.
- Assists students in developing long and short-term service goals and plans. Builds student competencies for accessing services independently and tracks progress on a regular basis.
- Facilitates student access to community partners and resources, including locating housing, food, clothing, school programs, vocational opportunities or services, mental health services; assist clients to develop resources and connect with social support networks.
- Coordinates and monitors services within Literacy Pittsburgh and with partner agencies, county human service providers, schools, healthcare, and government agencies.
- Creates comprehensive case notes and tracks client activities in relation to the service plan for outcomes monitoring and communication of client progress in interdisciplinary team meetings.
- Participates in staff development and training opportunities.
- Performs exit interviews to determine successes and future plans.
- Participates in the delivery of training to external service providers.
- Other duties as assigned related to the position and Literacy Pittsburgh staff responsibilities.
