Description

PURPOSE:
To facilitate collaboration and ongoing monitoring of the interdisciplinary support team of each youth served to ensure those residing in Intensive Intervention ChildBridge Group Homes receive multi-disciplinary, trauma-informed, comprehensive care services and planning. To initiate, facilitate, and perform a variety of hands-on and administrative casework related functions pertaining to youth assigned to their caseload, including but not limited to the development of the comprehensive treatment plan through ongoing clinical and professional consultation.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
1. Carries a caseload of youth who reside within Intensive Intervention ChildBridge Group Home(s) utilizing intensive (low youth to case             manager ratio), in-depth, person-centered, and assertive multi-disciplinary case management approaches across processes: screening         and intake, assessment and risk evaluation, planning, implementation, evaluation, and transition/discharge planning.
2. Reviews and maintains all youth records and history including: community support team assessments and plans, intake, consents and           medical providers, assessments, unusual and critical incidents, person centered and comprehensive care plans, clinical summaries,               medical diagnoses and appointments, education, legal, financial benefits,consents, monitoring and completing progress notes, and               securing all required documentation for assessment and provision of services to child.
3. Prepares files for regular audits including DCFS Licensing, BQM, HCBS, & CARF. Understands applicable rules, policies, and regulations      across Department of Human Services, Department of Child and Family Services, and Illinois State Board of Education as relevant.
4. As able, participates in clinical assessments of potential new youth intakes, & on-going assessments in collaboration with the clinical               supervisor and behaviorist assigned including but not limited to: Demonstrated competency in MOAS and SIT scales, and in                     Residential Behavioral Specialist skills and all youth behavioral intervention and skill acquisition plans.
5. Actively provides staff explanation, modeling, and feedback in relation to implementation strategies across goals and outcomes in                  collaboration with clinical and home management.
6. Scheduling and coordination of all interdisciplinary and/or case and family team meetings at varying intervals, as well as observations,          assessments, and interviews from external service providers, including Initial, quarterly, annual, and ongoing evaluation of service                  utilization, response to previous intervention(s), availability of/barriers to effective treatment.
7. In collaboration with Nursing team, ensures health and well-being of youth through scheduling and coordination of all medical,                        psychiatric,dental, vision, and specialty appointments, consultations, & subsequent follow-ups.
8. Ensures youth participation in recreational and community integration activities as it pertains to registration and consent (e.g., special             school outings, community, and summer camps, attending religious services, etc.).
9. Acts as primary contact for parents/guardians, special education teachers, outside stakeholders, and case and family teams. Attempts            and maintains regular, ongoing contacts with the parents/guardians of children. For DCFS wards, makes all efforts to establish a                    relationship with the biological family and assists the biological family in reaching reunification when applicable to the case.
10. Ensures that medical provider follow-up recommendations are completed and communicated to RN, Dietician, Home Manager, and                direct care team members as relevant.
11. On-Call: Participates in rotating case management on-call schedule outside regular business hours and weekend including: notifications         of unusual incidents, completion of DCFS database entry of incidents that occur during your on-call, facilitating communication with               hospitals during emergencies, on-site hospital advocacy as needed, and collaboration with RN to ensure paperwork and                                medication/script reconciliation occurs following return to CGH.
12. Attends and completes all DHS, HCBS, and DCFS required trainings.
13. Maintains state provided email, access to DCFS databases, and ensures youth confidentiality.
14. Responsible for all safety activities in your workplace and immediately report all accidents and safety hazards to your supervisor.
15. Provides crisis support to staff in assigned homes as needed, as well as provides emergency ratio staffing throughout the week as                needed and able.
16. Successfully completes Little City required training, re-training, and any additional training as required by Little City policies and                       practices, or as assigned by his or her immediate supervisor.
17. Other duties as assigned by your supervisor.

Qualifications


MINIMUM QUALIFICATIONS:
Must have a college or university degree that meets the federal educational standards required to serve or to be eligible to serve as a Qualified Intellectual Disability Professional (QIDP)1 and have at least one-year experience working directly with individuals with developmental disabilities, required. Must be at least 21 years of age. Must have excellent verbal and written communication skills as well as organizational skills. Must be capable of transporting children and possess a current IL valid driver’s license. Upon completion of on-boarding, must demonstrate competency in Trauma Informed Ukeru & Advanced CPI..

PHYSICAL REQUIREMENTS & WORK ENVIRONMENT:
While performing the duties of this job, the employee is subject to sitting, walking, and standing for prolonged periods; frequently grasps, lifts, holds, or feels objects; occasionally stoops, kneels, crouches, or crawls. The employee frequently is required to use manual and finger dexterity and eye-hand coordination when working with persons served and handling office, medical, or household equipment. The employee is subject to assist and lift up to 50 pounds with varying amounts of assistance on a reoccurring basis. The employee is required to have corrected vision and hearing within normal range and the ability to operate a motor vehicle. Must have the ability to perform CPR and CPI. Occupational exposure to blood, body fluids, infectious substances, including communicable diseases when performing personal care (Category III). Occasional exposure to cleaning agents and disinfectants. Residential work environment with a noise level of normal to loud.
