Description
The Health Services Support Worker will pass morning medication to various individuals at different locations. This worker will also ensure that the individuals who live at our Children's Group Homes are transported and arrive at their scheduled medical and specialist appointments promptly, as well as assist the individuals during the appointment process when necessary. This position has a schedule of Monday-Friday from 7:00 am - 3:00 pm with some flexibility required due to driving routes and traffic.
Essential Functions:
- Must be capable of reading, writing, and speaking English to plan, coordinate, and schedule with program staff and children, as well as guardians/caregivers (when necessary). Written communication must be clear, concise, grammatically correct, and free of spelling errors.
- Must possess sufficient strength, agility, and sensory acuity to support program participants safely and effectively during appointments. This includes responding to behavioral problems using agency-approved safety training in which the individual is displaying aggression towards themselves or others, or other emergencies using learned safety measures, with adults weighing 150 lbs. Must also assist with daily living tasks such as toileting and dressing need that occur during appointments.
- Must behave in a professional manner that reflects positively on themself and Little Friends Inc. This includes demonstrating a high commitment to customer service and customer satisfaction for all internal and external customers of Little Friends Inc.
- Must possess sufficient computer skills to document appointment notes and incident reports.
- Must be aware of, and follow, the policies and procedures of Little Friends Inc. and Residential services.
- Required to complete all training and re-certifications per Little Friends, Residential, and DHS.
SPECIFIC DUTIES:
- Pass medications to individuals at multiple locations. Completing ATW med pass before individuals leave for their day program.
- Assists the individuals to arrive promptly for scheduled medical and specialist appointments. This driving may transport the individual in an agency vehicle or personal car.
- Provides necessary direct support to each individual during the appointment to ensure that the individual understands the doctor’s instructions. This includes but is not limited to staying with an individual while he/she is being seen by the physician and/or dentist.
- Makes entries in the individuals’ case files as appropriate.
- Provides verbal and written information to the Nurse, Manager, and staff as needed to ensure appropriate follow-up treatment is offered to individuals, including but not limited to medication schedules and procedures required by physicians or dentists.
- When no Children's appointments are scheduled, staff may be required to facilitate adult appointments both scheduled and emergency.
- Attends in-service and training programs as a scheduled supervisor.
- Executes other duties assigned as required to assist in the provision of services within the CLS.

Requirements
- Must be a minimum of twenty-one years of age.
- Possess a High school diploma or the equivalent is required.
- A college degree in psychology, social work, special education, medical/hospital appointment/records, or human services is desirable
- Experience working with developmentally disabled individuals is preferred.
- Must have a valid State of Illinois driver's license, insurance, and a good driving record (No more than 2 accidents over the past 5 years as shown by Motor Vehicle Report - ran upon hire)
- Reliable transportation is necessary for this position.
