Behavioral Health Services Navigator
POSITION SUMMARY
Under the general supervision of the Director of Integrated Healthcare Services, the Behavioral Health Services (BH) Navigator supports the Behavioral Health and primary care providers, serving as a liaison between patients, LRMC staff, and community providers to ensure continuity of care and access to integrated healthcare services. The BH Navigator also educates individuals and families in the community on health education and disease prevention using a team-based model.
Responsibilities include managing behavioral health call queue phone calls, scheduling BHC and psychiatric appointments, assisting with MOUD medical appointment scheduling, confirming appointments, taking non-emergent phone messages clearly and concisely, and transferring emergency calls to the appropriate BHC and/or clinical team member. Additional duties involve tracking, coordinating, and communicating patient referrals for LRMC BH services, providing excellent customer service via phone, coordinating with external medical offices, and performing other assigned tasks.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned:
- Demonstrate the ability to establish and maintain effective, productive relationships with co-workers, supervisors, patients, and the community members in a manner consistent with the mission of LRMC.
- Answers the BH call queue and performs the greeting of patients and other callers in a respectful and compassionate manner in a clear, concise and complete manner promptly and courteously in accordance with Center’s procedures.
- Appointment Scheduling /Rescheduling for patient accordance with existing Center’s procedures.
- Strives to retrieve messages hourly and return calls to patients in a timely manner.
- Accurately and completely take messages and ensure the appropriate person immediately addresses emergency calls.
- Managing the referral process, including gathering and verifying patient information, and scheduling appointments.
- Responsible for tracking, coordination, and communication of patient referrals per policy.
- Assembles information concerning patient’s clinical background and other referral needs.
- Scheduling referral appointments and maintaining accurate records of referrals and follow-up activities.
- Monitor referral process to ensure compliance with applicable regulations.
- Maintains a neat/well organized work area.
- Ability to display flexibility as patient flow changes.
- Attending all in-service training as required.
- Maintains the patient and Center’s confidentiality and Code of Compliance per policy.
- Performs other duties as assigned.

EDUCATION and/or EXPERIENCE
- High school diploma or Equivalent is required, some college or college degree preferred.
- At least one year of experience in healthcare, social services or call center industry required.
- Bilingual English/Spanish preferred but not required.
- Knowledge of behavioral health and medical terminology
