We are excited to offer an up and coming vision for children with complex medical needs and services for children in this area. We specialize in children with special needs and helping meet their educational and medical needs. We understand how isolating and difficult it can be to find the right fit for medically needy children and we are happy to be here to help.
Job Summary: The Direct Care Provider provides direct care to center individuals under the supervision of a Registered Nurse and the Director of Nursing. Such care is provided in accordance with all professional, state and federal requirements.
REPORTS TO: The RN on duty
Duties and Responsibilities:
· Delivers competent and quality care in compliance with physician orders.
· Ongoing observation and assessment of individuals condition and needs.
· Takes appropriate action based on an individual's assessment.
· Documents individual care actions in an official record in response to medical needs.
· Identifies in writing individual's responses to treatment, specifically those that are unusual or unexpected.
· Monitors and interprets vital signs.
· Provides personal hygiene assistance.
· Monitors individuals I and O.
· Maintains adequate medical supplies.
· Assists with the development of an individual plan of care.
· Outlines individual goals and desired outcomes.
· Establishes individual care that is consistent with the medical plan provided by the physician.
· Collaborates in the identification of individual needs and/or problems.
· Reinforces individual's and family education for care of the individual.
· Intervenes as appropriate to prevent or reduce an individual's injury.
· Maintains the privacy and supports the rights of individuals and their families related to
· Communicates effectively with the family and company staff to provide appropriate information relative to medical care.
· Works in a collaborative manner with other health care personnel.
· Acts in accordance with the Nurse Practice Act while complying with company policy and procedure.
· Performs work in line with company mission and values.
· Maintains appropriate professional credentials and responsibility for personal and professional development.
· Other duties as assigned.
Physical Requirements:
· Must have transportation to arrive on time to the Center.
· Ability to stand, lift, or assist in lifting the individual for skilled nursing care, feeding, changing, bathing and emergency response.
· Must be able to bend, stoop, kneel, squat and generally demonstrate flexibility to meet the care needs of the individual.
· Must be able to see with visual and listen with hearing acuity to appropriately monitor the individual and associated medical equipment.
· Must have strength and manual dexterity, including grasping and manipulating small objects, to operate medical equipment appropriately.
· Must be capable of responding to emergency situations.
Qualifications:
· Florida licensed CNA, LPN, Medical Assistant, EMT, or Paramedic
· Two (2) years recent pediatric experience
· Engage in learning sessions (educationally) with the children
· Must effectively communicate with patients, caregivers/parents, RN case managers, staffing and medical records personnel.
· Current CPR certification;
· First Aid certification;
· Evidence of negative TB skin test or chest x‐ray; and
· Satisfactory criminal background check.
· This position is subject to drug screening at time of conditional job offer as a part of the Drug Free Workplace.
