Join our ever-growing rehab staff in a hospital-based outpatient clinic at one of the largest critical access hospitals in the region based on services provided. We have a large physician group consisting of primary care providers and specialists including orthopedics, women's health, interventional pain, spine surgery, and neurology. Enjoy a flexible schedule, paid documentation time, 1-hour one-on-one treatment and 1-hour initial evaluations, excellent continuing education and travel reimbursement, and paid National and State APTA dues. There is weekend inpatient coverage expected once every 9 weeks and rotating holiday inpatient coverage every year. We are looking for a therapist who puts the needs of their patients above all else, demonstrates accountability and integrity, and takes initiative in continuously striving to be the best provider they can be for their patients.

JOB: TITLE: Licensed Physical Therapist STATUS: Non-Exempt

DEPARTMENT: Rehabilitation Services - (Inpatient and Outpatient Clinic)

PRIMARY FUNCTIONS:

SUMMARY OF MAJOR FUNCTION

Licensed Physical Therapists are responsible for the delivery of skilled patient care in a safe and timely manner to all of those referred for services. The Physical Therapist understands and adheres to all hospital and departmental policies and procedures, as well as complying with the New Hampshire Physical Therapy Practice Act (Section 328-A:2).

ESSENTIAL FUCTIONS AND WORK ROLE RESPONSIBLITIES

CLINICAL OUTPATIENT DUTIES

Physical Therapy practice is based on the New Hampshire Physical Therapy Practice Act, Section 328-A:2, which includes:

  1. Clinical Evaluation and Assessment: Conduct comprehensive evaluations and assessments of patients' musculoskeletal and neuromuscular conditions, pinpointing functional limitations and developing individualized treatment plans in accordance with New Hampshire's physical therapy regulations.

  1. Treatment Planning and Implementation: Formulate and execute evidence-based physical therapy treatment plans tailored to each patient's specific needs, promoting optimal rehabilitation and functional improvement while adhering to state practice guidelines.

  1. Rehabilitation and Functional Restoration: Focus on restoring patients' physical function, mobility, strength, and coordination through therapeutic interventions, contributing to their overall well-being and independence.

  1. Patient Education: Educate patients and their families on prescribed exercises, self-care strategies, and injury prevention techniques, ensuring that patients are well-informed and capable of managing their conditions effectively.

  1. Collaboration and Communication: Collaborate closely with an interdisciplinary healthcare team, including physicians, nurses, occupational therapists, and other healthcare professionals, to ensure coordinated patient care and successful treatment outcomes, in alignment with the collaborative practices emphasized in New Hampshire's physical therapy regulations.

  1. Inpatient Care: Provide physical therapy services in inpatient settings as needed, such as acute care, emergency department, same-day surgery, obstetrics unit, and/or the intensive care unit, actively participating in patient rounds and discharge planning to facilitate smooth transitions.

  1. Documentation and Record Keeping: Maintain thorough and accurate patient records, including evaluation findings, treatment plans, progress notes, and discharge summaries, ensuring that documentation is compliant with the state's physical therapy regulations and legal requirements.

  1. Continuing Education and Professional Growth: Engage in ongoing professional development to stay current with the latest advancements in the field of physical therapy, attending workshops, seminars, and relevant courses to enhance your clinical skills and knowledge.

  1. Compliance and Regulatory Adherence: Strictly adhere to all relevant laws, regulations, and ethical standards governing the practice of physical therapy in New Hampshire, upholding the highest levels of professional and ethical integrity.

REPORTS TO: Director of Rehabilitation Services

EXTERNAL AND INTERNAL CONTACTS: Patients and families, Hospital Personnel, Physicians and office Staff, Financial and Legal Representatives, Community Members, Contracting Agencies, Students and Academic Coordinators, Home Health, and Co-workers

SUPERVISES: A licensed Physical Therapist supervises the care given by a Licensed Physical Therapy Assistant and a Physical Therapy/Physical Therapy Assistant Student. A Physical Therapist also directs and supervises all tasks designated to a Rehabilitation Aide and other support personnel.

QUALIFICATIONS:

Education:

  • Must have degree in Physical Therapy from an accredited school (minimum requirement is Bachelor of Science)
  • Must have a current license or be eligible for licensure in the State of New Hampshire
  • 24 contact hours of continuing education as designated by the New Hampshire Licensing Board - every 2 years

Experience:

  • Previous outpatient experience preferred
  • BCLS required

WORK ENVIRONMENT: Works primarily in hospital and outpatient clinic areas with occasional work in community areas. Exposure to heat, cold, and hazards including communicable diseases and blood borne pathogens due to patient conditions and equipment.
