Care Coordinator
LIV Health is looking for an experienced, motivated, outgoing Care Coordinator in Laramie County. The position is open to an individual with a high school diploma and a minimum of one year experience in health or human services. Job duties include traveling in and around Laramie County and providing home visits. Other duties include:
- intakes, assessments, and screenings
- making referrals and care coordination with community partners
- assisting patients with paperwork and accessing community resources
- coordinating medical care
- attend staff meetings and virtual meetings

Pay: $19.00 - $23.00 per hour
Expected hours: 36 – 40 per week
