What are the responsibilities and job description for the Social Work Case Manager position at LIV Health?
Are you a motivated individual who likes working independently? LIV Health is looking for an experienced, motivated, outgoing individual to provide case management services in Albany County. The position is full-time and open to an individual with a BSW, CSW or other Bachelor of Human Services degree. Must be willing to travel and work from home with equipment provided. Job duties include meeting with patients in their homes or other community settings, completing intakes/assessments/screenings, making referrals, assisting patients with paperwork, accessing community resources and managing patients on the CCW and DDW Medicaid Waiver programs. Must have a knowledge of community resources, such as Medicaid, SSDI, SSI, SNAP, CHA, waiver programs, and medical social work. Must be able to work with interdisciplinary team and community partners. Case manager will be expected to attend staff meetings, phone meetings, community meetings and have good time management skills.
