What are the responsibilities and job description for the Social Work Case Manager position at LIV Health?
LIV Health is looking for an experienced, motivated, outgoing individual to join our case management team. This position is full-time and open to applicants with a BSW and or a CSW, or other Human Services degree.
Job duties include meeting with patients in their homes or in office, completing intakes/assessments/screenings, making referrals, assisting patients with paperwork, accessing community resources and managing patients on Medicaid and Medicare programs. Must be able to work with interdisciplinary teams and community partners.
Must have several years of case management experience and sound knowledge of community resources, such as Medicaid, Medicare, SSDI, SSI, SNAP, CHA, waiver programs, and medical social work.
