Job Description :
Bedrock Rehabilitation and Nursing Center at Suwannee is seeking an experienced and dedicated RN MDS Coordinator to join our team. The MDS Coordinator will be responsible for overseeing the development and completion of the Resident Assessment (MDS), ensuring compliance with all applicable state and federal regulations. The ideal candidate will have strong clinical assessment skills and be proactive in coordinating care plans to ensure the highest quality of care for our residents.
Key Responsibilities :
- Lead the development and completion of MDS assessments, ensuring compliance with state and federal guidelines.
- Update and maintain policies and procedures for MDS assessments and care plans.
- Perform administrative duties such as completing medical forms, reports, and evaluations.
- Develop and implement a quality assurance program for assessments and care plans.
- Collaborate with the Discharge Planning Coordinator on care plan sections for discharge plans.
- Participate in surveys and inspections conducted by authorized government agencies.
- Monitor Quality Indicators (QI) and Quality Measures (QM), implementing corrective actions as necessary.
- Ensure the timely completion and transmission of assessments, addressing any issues with the Administrator.
- Assist in scheduling and coordinating interdisciplinary care plan meetings.
- Work collaboratively with outside care team members, including families and residents' representatives.
- Contribute to therapy services and assist occupational therapists with activities.
- Assist in evaluating residents’ abilities and limitations to ensure appropriate therapy and activities are scheduled.
