Facility: Bedrock Rehabilitation and Nursing Center at Suwannee
Location: Live Oak, Florida
Shift: 8:00 AM - 5:00 PM
Salary: Up to $36 per hour (depending on experience)
Job Description:
Bedrock Rehabilitation and Nursing Center at Suwannee is seeking a compassionate and dedicated Social Services Director to join our team. In this role, you will implement the facility's social services programs under the direction of the Administrator and Director of Social Services. You will coordinate activities with the interdisciplinary team (IDT) to ensure residents receive the highest standard of care and services.
Key Responsibilities:
- Assist residents, families, and representatives with discharge planning, financial assistance, and referrals to community agencies.
- Ensure residents receive necessary behavioral health care in line with their care plans, promoting their physical, mental, and psychosocial well-being.
- Participate in resident/group council meetings and provide information on resident rights, living wills, etc.
- Stay up-to-date with federal and state regulations and professional standards to recommend changes to facility policies and procedures.
- Provide translation assistance for residents with limited English proficiency (LEP).
- Maintain a library of necessary materials, laws, and standards for compliance with current regulations.
- Collaborate with medical, nursing, and administration staff to plan and implement social services programs and activities.
- Participate in facility inspections and assessments by authorized government agencies as required.
- Assist in obtaining resources from community agencies to support residents’ needs.
Requirements:
- Master’s degree in Social Work (MSW), Sociology, Psychology, Gerontology, Special Education, or a related human services field required.
- Minimum of 1 year of supervised social work experience in a healthcare setting, working directly with individuals.
