LiveOnNY is currently looking for Family Support Advocate (Overnight)- Fulltime!
Do you possess the attributes to be a successful Family Support Advocate and perform the following essential functions?
Responsibilities :
The Family Support Advocate : provides on-site responses and leads family donation conversations by advocating for organ donation and supports families throughout the process. At its core, Family Support Advocates strive to meet and or exceed the monthly authorization goal to improve Organ Donation through education, supportive services, and relationship building with key hosptial stakeholders.
- Delivers high-quality bereavement support to potential donor families.
- Facilitates the opportunity for donation by keeping hospital staff informed regarding the plans and progression of the referral process.
- Offers donation in an empowering and inspiring manner. Responsible for educating potential donors Next-of-Kin (NOK) about donation options, status of donor designation, or requesting authorization for donation from legal NOK
- Obtains authorization or disclosure with first person authorization and conducts a uniform donor risk assessment questionnaire (UDRAI) with appropriate NOK / surrogate of the potential organ donor.
- Assess families understanding of grave prognosis and brain death declaration.
- Supports donor families by attending OR in Donation After Circulatory Death Donation (DCDD).
- Works in strong collaboration with hospital personnel, LiveOnNYs Family Service Lead (FS Lead), LiveOnNYs Administrator-on-Call (AOC), Referral Administrator (RA), and clinical staff to evaluate and determine the best time to discuss the opportunity for donation with potential donor families.
- Completes accurate, and timely documentation of donor-related activities in computerized confidential donor record, and forms according to established policies and procedures.

Must be able to work from 11pm-8am, 5 days a week.
This role requires frequent travel (valid driver license and own vechicle) and ability to commute to different hospital partners within our service area

Salary Range : $70,000 -$103,000
