Position Overview:
As a Care Services Coordinator at Live Well, you will play a pivotal role in the lives of our clients and their families. You will be responsible for conducting client assessments, developing customized care plans, and serving as the primary point of contact between families, caregivers, and the agency. Your goal is to ensure that each client receives the best possible care while fostering strong relationships with both clients and referral partners.
Key Responsibilities:
- Client Assessments: Conduct comprehensive assessments for new clients, developing personalized care plans tailored to their individual needs.
- Care Plan Management: Monitor and update care plans regularly to reflect changes in clients’ conditions, ensuring caregivers and families are kept informed.
- Family Communication: Act as the primary liaison for clients' families, offering regular updates on care and addressing concerns in a timely, compassionate manner.
- Referral Relationships: Build and maintain strong relationships with healthcare professionals and referral sources to ensure a steady flow of new clients and continued business growth.
