What are the responsibilities and job description for the Hospice Nurse Practitioner position at Live Well Hospice, LLC?
- Summary of Responsibilities: The Nurse Practitioner is responsible for providing nursing expertise, managing all services needed as determined by the Plan of Care and Medicare Conditions of Participation. He/she provides face-to-face clinical assessments and reports findings to the IDT.
- Principle duties and accountabilities include, but are not limited to:
- 1. Performing face to face assessments of all hospice patients to determine the continued eligibility of that hospice patient prior to the 180-day recertification, and prior to each subsequent recertification.
- 2. Reports to the attending physician and hospice medical director any significant changes in the patient’s condition and requests change of orders as needed.
- 3. Completes all required documentation.
- 4. Participates in admissions as necessary.
- 5. Complies with the state and federal regulatory requirements.
- 6. Other duties as defined.
- Qualifications:
- 1. Nurse Practitioner with current state license.
- 2. Complies with acceptable professional standards and practice.
- 3. Demonstrates good communication, negotiation, and public relations skills.
- 4. Requires reliable transportation, current driver’s license and current auto insurance.
