Overview
Livi Home Care is seeking a dedicated and compassionate PRN Registered Nurse to conduct client assessments and ensure high-quality care for our home care clients. Responsibilities
- Perform in-home client assessments to determine care needs and develop individualized care plans.
- Evaluate clients’ health conditions, functional abilities, and home environments.
- Provide recommendations for care services based on assessment findings.
- Collaborate with caregivers and families to ensure proper implementation of care plans.
- Maintain accurate and timely documentation of assessments and care plans.
- Educate clients and families on best practices for home care and health management.
- Ensure compliance with state regulations and Livi Home Care policies.
Qualifications
- Active Registered Nurse (RN) license in North Carolina.
- Minimum one year of experience in home care, hospice, or a related field preferred.
- Strong assessment, communication, and critical thinking skills.
- Ability to work independently and travel to clients' homes as needed.
- Reliable transportation and a valid driver’s license.
