Our Community Case Managers play a crucial role in supporting individuals with IDD to achieve their goals and live fulfilling lives.After 90 days remote flexibility is available.
In this role, you will:
- Partner with individuals with IDD and their families to develop individualized service plans.
- Advocate for their needs and ensure they have access to the resources and support they deserve.
- Facilitate connections to community services and programs that promote independence and inclusion.
- Monitor progress and make adjustments to service plans as needed.
- Provide ongoing support and encouragement to help individuals achieve their full potential.

QUALIFICATIONS:
- Bachelor’s Degree from an accredited four (4) year institution of higher learning with a specialization in psychology, mental health and human services, behavioral health, behavioral sciences, social work, human development, special education, counseling, rehabilitation, sociology, nursing or closely related field, OR
- Bachelor’s Degree from an accredited four (4) year institution of higher learning in an unrelated field and at least one (1) year of full-time equivalent relevant human services experience
- Strong communication, collaboration, and interpersonal skills.
- Excellent organizational and time management skills.
- A commitment to providing person-centered care.
- A valid driver's license and reliable transportation (required).
- Must pass comprehensive background checks per state and company regulations/policies.
