Position Overview: Living Water Clinic is seeking an ECM Director to oversee and manage the Enhanced Care Management (ECM) program in compliance with CalAIM standards. This leadership role is responsible for ensuring the effective delivery of services aimed at improving health outcomes for the ECM population. The ECM Director will play a critical role in driving program excellence, maintaining operational efficiency, and fostering a collaborative team environment.
Responsibilities:
- Develops, implements, and manages the workflows, policies, procedures of the
- ECM program. Ensures processes meet CalAIM and health plan requirements.
- Leads the ECM program operations, including hiring, training, and supervising
- care team staff.
- Partners with health plans and community organizations to coordinate ECM
- services and drive better health outcomes.
- Analyzes ECM performance data, identifies opportunities for improvement, and
- implements corrective actions as needed.
- Oversees development of program budget and ensures effective utilization of
- resources.
- Ensures collection and reporting of accurate data for claims, encounters, and
- member metrics.
- Conducts internal audits of billing, claims, and workflows to ensure compliance.
- Stays current on CalAIM regulations, best practices, and emerging value-based
- programs.
Requirements:
- Bachelor's degree in healthcare, social services, or related field required.
- 5 years experience in care management or population health programs.
- Experience with Medicaid programs preferred.
- Strong leadership, project management, and communication skills.
- Detail-oriented with the ability to manage multiple priorities.
