Position Overview:
Living Water Clinic-Clinica Agua Viva is seeking a dedicated and patient-focused Primary Care Physician with training in Family Medicine (FM) or Internal Medicine (IM) to join our team. The ideal candidate will provide comprehensive healthcare services, build lasting relationships with patients, and develop personalized care plans based on medical history and current health needs.
As a Primary Care Physician, you will play a pivotal role in delivering high-quality, patient-centered care to an underserved community while having the opportunity for professional growth and competitive earning potential.
Responsibilities:
- Provide medical leadership and collaborate with a multidisciplinary team to ensure integrative patient care.
- Diagnose, treat, and manage acute and chronic conditions for a diverse patient population across all ages.
- Offer direct patient care at the clinic and outreach locations within the Tulare County service area.
- Supervise and support physician assistants and nurse practitioners in clinical practice.
- Coordinate continuity of care for hospitalized patients and those transitioning from medical respite care.
- Assist in developing and optimizing electronic health record (EHR) templates and the patient portal.
- Participate in quality improvement initiatives, ensuring compliance with clinical standards and best practices.
- Engage in community outreach and education, representing Living Water Clinic’s mission and values.
- Participate in management meetings and compliance discussions as part of a collaborative leadership team.
- Develop and implement medical policies and procedures, ensuring integration with other healthcare programs.
- Complete all required training within the first 30 days of employment.
- Uphold Living Water Clinic-Clinica Agua Viva’s policies and procedures to maintain a high standard of care.
Requirements:
- Medical License: Must hold a valid medical license from the Medical Board of California or the Osteopathic Medical Board of California.
- Board Certification: Board-certified or board-eligible in Family Medicine (FM) or Internal Medicine (IM).
- DEA & Furnishing License: Valid DEA registration and ability to prescribe medications.
- BLS/CPR Certification: Must be current.
- Bilingual in Spanish and English preferred but not required.
- Strong diagnostic, communication, and problem-solving skills.
- A compassionate and patient-centered approach, with a commitment to improving community health.
- Ability to work both independently and collaboratively within a healthcare team.
- Experience with electronic health records (EHR) and medical coding systems is a plus.
