While this position performs all the duties of a Case Manager it is distinguished from it by the fact that the incumbent works with a caseload of 20 to 30 clients and requires more frequent client contacts, a minimum of two monthly contacts. Work is performed in the office and in the field. Supervision of others is not a responsibility of this class. The incumbent works under the clinical supervision of a qualified mental health professional. A Supportive Case Manager does related work as required. Examples of Duties Identifies clients in need of supportive case management services by completing assessment forms with the client applying for mental health services; Aids the individual client in achieving objectives and maximizing independence and productivity through support, training and assistance in use of personal and community resources; Coordinates and participates in the development of an individualized, goal oriented service plan; Assures assessment of clients on their caseload in order to have their goals, strengths and skill deficits identified;Provides outreach to clients and families when appropriate who are in need but are not currently using mental health services by identifying options that are responsive to their needs and preferences; Monitors and follows up the implementation of the written service plan by linking clients with necessary providers, assisting in gaining access to needed medical, psychiatric, social, educational and other services, and maintaining contact throughout the course of treatment;Reduces inpatient treatment by providing whatever services or support is necessary to prevent or resolve crises; Assures medication regimen compliance through teaching and interpreting to clients the importance of prescribed medications, monitoring usage and refilling of prescriptions and being aware of and alert to side effects; Acts as the head of the Single Point of Access (SPOA) Committee, reviews all SPOA referrals, schedules SPOA Committee meetings, documents SPOA Committee recommendations and works with referral sources, referred clients and various community resources;Aids in developing formal and informal community supports and networks of relatives, friends and others; Assists clients in learning to use fiscal resources through budget planning, monitoring and instruction in spending and assisting with applications and entitlements; Provides outreach to engage clients and helps clients to achieve their own goals concerning where and how to live; Maintains contact with emergency services;Maintain waitlists and act as liaison between referred clients and service providers;Track and report data as assigned; and Performs other related duties as required.PERFORMANCE KNOWLEDGE, SKILLS, ABILITIES AND PERSONAL CHARACTERISTICS : Good knowledge of case management techniques; good knowledge of the problems of the chronically mentally ill; good knowledge of human behavior; good knowledge of the community resources available for the client population and how to effectively access them; working knowledge of counseling and crisis intervention techniques; ability to analyze and organize date and prepare records and reports; ability to keep accurate, clear, legible records and make reports; ability to maintain client records in accordance with State and agency rules and regulations; ability to understand and follow complex oral and written instructions; ability to communicate effectively both orally and in writing; ability to establish and maintain effective working relationships with others; good powers of observation and analysis; physical condition commensurate with the demands of the position. Typical Qualifications Applicants must meet all requirements for one of the following qualification options in order to be approved for examination and / or appointment : Qualification Option # 12Training / Education Possession of a Bachelor's degree from a New York State or regionally accredited college or university which included 18 credit hours in a human services fieldPossession of an Associate's degree from a New York State or regionally accredited college or university in a human services field(1).General experience One year of experience in providing direct services to persons diagnosed with mental disabilities ; OROne year of experience in linking persons diagnosed with mental disabilities to a broad range of services essential to successfully living in a community settingThree years of experience in providing direct services to persons diagnosed with mental disabilities (2); ORThree years of experience in linking persons diagnosed with mental disabilities to a broad range of services essential to successfully living in a community setting3.Supervisory experience N.A.N.A.License / Certification Possession of a valid New York State driver's license. Part-time service will be given prorated credit toward experience requirements.For purposes of these minimum qualifications, a human services field shall include social work, psychology, nursing, rehabilitation, education, health education, occupational therapy, physical therapy, recreation or recreation therapy, counseling, community mental health, child and family studies, speech and hearing.For purposes of these minimum qualifications, the term mental disabilities refers to persons properly diagnosed as suffering from mental illness, mental retardation, alcoholism or substance abuse.For purposes of these minimum qualifications, this shall include medical, psychiatric, social, educational, legal, housing and financial services. Supplemental Information An appropriate driver's license must be maintained throughout employment.A driving record evidencing safe and responsible vehicle operation is required. Must authorize and execute any necessary releases to enable the appointing authority to verify licensure and driving history involving violations and / or crimes. Supportive Case Manager, C in all divisionsPROVISIONAL APPOINTMENT NOTICE : An individual who receives a provisional appointment must take the examination for the position title on the next occasion it is offered. In order to be eligible for a permanent appointment to the position, the provisional appointee must pass the exam and be ranked 1, 2, 3, or be tied in score with the individual ranked number 3 of those who are willing to accept appointment to the position. It should be understood that even if the provisional appointee is eligible for appointment, the employer may choose another candidate to fill the position on a permanent basis.
