Livingston HealthCare Clinical Rotations and Internships
Students interested in completing a clinical or internship rotation with Livingston HealthCare must submit this application. To expedite the process, it is recommended to upload required documents with your application. These include:
- Letter of Good Standing from Institution
- Outlining up-to-date immunizations, including TB test
- Noted clearance of a background check and drug panel screening within 1 year of starting rotation
- Proof of COVID vaccination or statement of declination of vaccine
- Pertinent requirements of rotation or internship for LHC preceptors to review
- Syllabus
- Project Guideline
