POSITION SUMMARY:
Is directly responsibility for filing and processing records promptly and accurately. Works with the realization that errors in filing information could affect patient progress or have a significant impact for the hospital.
PRIMARY RESPONSIBILITIES & AUTHORITIES:
- Assembles content of inpatient medical record into established order for permanent filing. Reviews inpatient, outpatient, and emergency room records to ensure that required reports and signatures are included in the records.
- Checks on a routine basis for the existence of any prior medical record of every patient admitted as an inpatient.
- Performs admission procedures daily along with daily statistics.
- Files all loose reports in the proper location.
- Maintains unit numbering system for all records except lab reports.
- Performs record release for insurance requests, attorney requests and patient requests.
- Scanning of medical record documents into the electronic medical record.
- Adheres to all hospital and departmental policies.
OTHER DUTIES AND RESPONSIBILITIES:
- Screens all incoming calls directed to the department.
- Assists in retrieving charts for Performance Improvement studies for the various departments. Assists in retrieving charts for the QSource abstractions, Anthem Survey submissions and the Trauma Registry.
- Must be able to perform work with a high level of accuracy.
- Other duties as assigned.
MINIMUM QUALIFICATIONS (EDUCATION, EXPERIENCE, SKILLS, ABILITIES):
High school education or equivalent.
May have completed vocational training for Certified Medical Secretary.

PHYSICAL DEMANDS:
May remain seated for extended periods of time. Some walking for taking charts to the medical units is required. Ability to stand, bend and stretch is required for filing. Use of the hands for taking notes, typing, and message is required. Some lifting required.
WORK ENVIRONMENT:
Well lighted, ventilated area within the hospital.
