LivWell Community Health Services is a non-profit organization providing comprehensive health and support services, education, and advocacy, as related to HIV/AIDS, sexually transmitted diseases, and viral hepatitis. Quality health care is provided to persons living with HIV/AIDS or sexually transmitted diseases, support services are provided to HIV clients and their families, and prevention education and testing are provided to all clients, their families, and the community at large.
Position Overview
The Support Services Manager oversees the Support Services department operations and collaborates with the Executive Director to provide guidance to the Support Services team beneath the housing umbrella of services. The Support Services Manager is proactive in creating person-centered departmental protocols to assist HIV-positive individuals in obtaining appropriate supportive housing services, and assuring compliance with all programmatic, regulatory, and licensing requirements which affect the HOPWA and other housing programs.
Key Responsibilities:
· Plan and direct the day-to-day operations of the Housing Support Services department
· Lead and supervise a team of housing Case Managers and Navigators, providing guidance, support, oversight, and setting high-performance standards for proper intake processes for new clients, and including eligibility requirements
· Maintaining departmental adherence to agency goals, objectives, standards of performance, and all policies and procedures
· Coordinate intake assignments and distribute client workloads equitably and timely to Case Managers and Navigators
· Manage and review the documentation process and ensure proper case notes and information are documented within EHRs and/or departmental systems.
· Develop results by communicating expectations, planning, monitoring, appraising, and providing learning opportunities to team members
· Arrange and conduct regular meetings with housing staff to communicate and execute any changes, new information, and/or project statuses
· Assess departmental effectiveness, and develop new plans, policies, and strategies for the housing department to achieve organizational goals and growth.
· Collaborate with other departments within the agency to ensure the integration of housing services with other programs and resources
· Establish and maintain effective relationships with external stakeholders, including government agencies, community partners, and housing providers
· Ensure HOPWA, HUD, and other housing grantor reports are completed and submitted to the appropriate agencies in a timely manner
· Participate in the research and preparation of grant proposals to fund housing client services and in the ongoing assessment and evaluation of received grants
· Participate in the development of the departmental budget in collaboration with the Executive Director
· Update or assist in update and revisions of the LivWell CHS Housing Case Management Policies and Procedures and handbook as needed
· Serve on the Quality Management Team and attend meetings as necessary
· Supporting Case Management staff by contacting clients, assessing complex issues, and creating individual approaches to solutions if needed.
· All other duties as assigned
Skills Needed:
· Knowledge of HIV and STIs, preferred;
· Proven leadership and people management abilities;
· Ability to set clear team goals, motivate team members, and evaluate progress towards goals;
· Knowledge of current social, economic and health trends and services, and the ability to adapt trends to practical program applications and link clients with services needed;
· Ability to conduct interview/assessments, summarize and interpret findings;
· Ability to communicate complex information effectively, with a wide variety of people;
· Ability to establish and maintain effective relationships with diverse populations;
· Knowledge of current office practices and equipment and work/project scheduling practices;
· Discreetly handle private and sensitive matters and materials with a high level of confidentiality.
Required Qualifications
· Bachelor’s degree in Human Services, Social Work, Psychology, Sociology, Nursing, or a similar field
· Two-years’ experience in social work or a human service related field
· Knowledge of housing regulations, policies, and funding sources, including affordable housing programs, fair housing laws, and housing development processes
· Valid driver’s license and are insurable to drive LivWell CHS’ vehicles
· Experience with basic technology and office computer programs, such as Microsoft Office Suite
Preferred Qualifications
· Master’s degree or higher in Social Work, Human Services, or a related field
· Degree program accredited by The Council on Social Work Education (CSWE)
· Current LSW or CSW
· Experience in housing program management, preferably in a nonprofit or government setting
