What are the responsibilities and job description for the Community Health Worker position at LMAS District Health Department?
The LMAS District Health Department seeks a Community Health Worker (CHW) for our UP Mom Program. This position is full-time with benefits, and will be based in one of our four offices; Newberry, St. Ignace, Munising or Manistique. The CHW will provide health and human services for pregnant mothers and infants with a history of current, or risk for, substance use disorders.
A high school diploma or equivalent is required. CHW credentials, and experience with social services, substance use disorders, and public health are preferred but not required. CHW credentials will be obtained upon hire. Possession of a valid Michigan driver's license, reliable transportation, and the ability to travel are required.
Job Type: Full-time
Pay: $16.03 - $19.88 per hour
Expected hours: No less than 40 per week
