Job Summary :
We are seeking a Patient Coordinator to join our transplant team, providing continuity of care for patients and ensuring efficient scheduling and coordination of services. The Patient Coordinator will work closely with clinical, financial, and data management teams to minimize scheduling complications and provide exceptional customer service. This role requires attention to detail, strong organizational skills, and the ability to problem-solve in real time.
Key Responsibilities :
- Monitor patients throughout various transplant phases to ensure completion of required testing and services.
- Schedule and authorize patient services efficiently, minimizing unnecessary visits to the medical center.
- Validate financial clearance for scheduled services and communicate with patients regarding appointments.
- Coordinate with ancillary departments and clinicians to ensure seamless care delivery and testing completion.
- Collect and manage testing results, provider notes, and other relevant records for patient care.
- Address non-compliance issues with patients and alert clinicians as needed.
- Actively participate in team meetings, complex case discussions, and process improvement initiatives.
- Analyze patient information, charts, and physician notes to schedule care appropriately and communicate outcomes effectively.
