Position Summary
Under the leadership of Oncology, the Cancer Patient Navigator functions as a member of a multidisciplinary team, serving as an advocate and educator to cancer patients from diagnosis through end of active treatment, and possibly end of life. The Cancer Patient Navigator offers assistance to patients, families and caregivers to help overcome system barriers and facilitate timely access to quality care. Patient navigation includes assessment of physical, psychological and social needs of the patients.
Responsibilities:
- Review lung Radiology reports and work with lung navigation software
- Facilitates referrals to Pulmonology for those patients with positive lung nodules.
- Ensure patients follow-up with ordered procedures.
- Assist in the patient’s understanding of their diagnosis, treatment options, and resources available, including any appropriate clinical research studies and facilitating any surgical or medical oncology referrals as ordered by the physician.
- Serving as a link between patients and other care providers, including social work and dietitians, radiation oncology, coordinating multidisciplinary case conferences, and education of staff in the Radiology and Oncology departments.
