Lock Hearts Home Care Solutions is seeking an experienced House Manager to join our team!

Job Summary:
The ideal House Manager will possess open availability, patience and experience. The duties of the House Manager include but are not limited to medication reminders, personal care and daily activities of living.
JOB DUTIES: Coordinates a wide variety of programs and activities for individuals with special needs. Develops, implements, and evaluates individual service needs of clients, which promote independence and the achievement of individual outcomes as defined in the Individual Service Plan (ISP).
PRIMARY DUTIES: Under the supervision of the Director/Assistant Director, works with staff to plan and implement methods and activities that will ensure the appropriate curricula are implemented to meet needs of developmentally and physically needs of individuals; evaluates and plans activities, provides advice and guidance as appropriate. Transports clients to doctor visits, assist with meal planning and visits, as needed.
Essential Functions:
· Supervise staff, ensure that all duties are carried out in accordance with company policies and procedures.
· Ensure that all safety policies and procedures are followed at all times.
· Monitor staffing levels to ensure that staffing needs are met in a timely manner.
· Ensure that quality patient care is maintained.
· Responsible for the orientation of new staff, ensuring that they have been thoroughly reviewed and trained in all policies, procedures, and standards required by Lock Hearts Home Care Solutions.
· Assist with direct care coordination, including but not limited to; medication reminders, medication tracking, discharge planning, etc.
· Maintain accurate patient records and report on a daily basis to the appropriate departments within the facility management company.
· Perform additional duties as assigned.
EXPERIENCE: Must have at least 12 months of experience working with individuals with disabilities in a home-like setting & 12 months of verifiable managerial experience.

REQUIRED LICENSES OR CERTIFICATIONS:
- Driver’s License
- CPR/First Aid
- Clean BCI Background Check
- Med Cert Category 1
- GED or equivalent
- Must have a personal vehicle with automobile insurance and valid driver's license with less than 6 points
