Responsibilities And Duties
- Conduct geriatric intake assessments and manage medications for PACE program participants
- Perform comprehensive health histories and physical examinations
- Provide patient education and counseling
- Order, interpret, and follow up on laboratory tests and results
- Document patient care thoroughly using PACElogic electronic health record system
- Initiate emergency measures as needed
- Recognize and respond to abnormal findings
- Collaborate with the healthcare team to ensure optimal patient outcomes
- Participate in on-call rotation (1 week every 5 weeks) for triage services
- Adhere to all clinic policies and procedures
- Maintain up-to-date knowledge of geriatric and family practice best practices
- Active North Carolina state license
- Current certification as a Family Nurse Practitioner
- Experience in geriatric care and medication management
- Familiarity with PACE programs or similar elderly care initiatives
- Proficiency in electronic health record systems, preferably PACElogic
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
- Willingness to participate in on-call rotation
