AMB: Diabetes Treatment Center-LLUMC (TEMPORARY)- Part-Time, Days

Job Summary: The Educator-Diabetes is a Registered Nurse (RN) that serves as a clinical expert and demonstrates advanced knowledge of pre-diabetes, type1, type 2 and secondary diabetes treatment and management. Provides patient-centered whole person care and education for inpatients admitted to Loma Linda University Medical Center, East Campus Hospital and Surgical Hospital and also for outpatients served at the Diabetes Treatment Center. Works closely with the Director and Medical Director to acquire and maintain American Diabetes Association (ADA) recognition for outpatient diabetes education program and Diabetes Prevention Program recognized by the Centers for Disease Control. Advances the DTC as a center of excellence and resource providing cutting-edge diabetes prevention and care to the internal and external community of healthcare providers and patients. Performs duties within RN scope of practice and in accordance with policies, procedures, standards of practice and regulatory requirements. Responds to patient referrals and effectively manages a multidimensional caseload ensuring high-risk diabetic patients are prioritized appropriately. Continually expands knowledge base and stays current with evolving best practices, clinical practice guidelines, diabetes medications, disease state, patient tools and technology. Utilizes disease-specific knowledge base and the nursing process (assessment, planning, and intervention, evaluation) independently to evaluate patient and caregiver level of understanding of diabetes, treatment, complications and self-management of the disease. Confers with applicable healthcare team members to develop an individualized education plan with learning objectives that support patient and caregiver engagement and confidence to self-manage diabetes on a day-to-day basis. Provides individual and group patient and caregiver education that aligns with current American Diabetes Association (ADA) curriculum and includes, but is not limited to: medication management (insulin and non-insulin injections and oral medications), glucometer training, continuous glucose monitoring and data download, clinical signs and symptoms of hypo and hyperglycemia and appropriate responses, A1c lab tests results and glycemic control significance, coping with the disease and managing risk factors and community resources such as outpatient Diabetes Treatment Center and support groups. Promotes lifestyle modification and behavior changes that prevent short and long term complications related to diabetes. Establishes a credible and dependable working relationship with healthcare providers and serves as a resource for diabetes education and self-management of disease. Functions within the expectations of an accomplished professional nurse and meets nursing excellence standards to achieve and maintain Magnet Recognition as outlined by the American Nurses Credentialing Center. Excels in implementing new knowledge, innovations and improvements in practice to enhance the patient experience and clinical outcomes. Performs other duties as needed.

Education and Experience: Bachelor's Degree in Nursing required. Minimum three years of nursing experience which has provided the opportunity to demonstrate competencies in clinical nursing caring for diabetic patients required.

Knowledge and Skills: In-depth knowledge of diabetes treatment and management including, but not limited to: medications, diet, exercise, blood glucose monitoring, equipment and supplies required. Understanding of criteria set forth by the ADA for outpatient diabetes education program recognition and by the CDC for Diabetes Prevention Program. Solid concept of health promotion and disease management strategies required. Must have exceptional ability to engage and motivate others. Substantial knowledge of education principles for all ages required. Able to: read; write legibly; speak in English with professional quality; use computer and software programs necessary to the position; troubleshoot and calibrate patient care equipment. Able to relate and communicate positively, effectively, and professionally with others; be assertive and consistent in following and/or enforcing policies; work calmly and respond courteously when under pressure; lead, supervise, teach, and collaborate; accept direction. Able to communicate effectively in English (and Spanish preferred) in person, in writing, and on the telephone; think critically; work independently with minimal supervision; perform basic math functions; manage multiple assignments effectively; work well under pressure; problem solve; organize and prioritize workload; recall information with accuracy; pay close attention to detail. Able to distinguish colors and smells as necessary for patient care; hear sufficiently for general conversation in person and on the telephone; identify and distinguish various sounds associated with the work place/patient care; see adequately to read computer screens, medical records, and written documents necessary to position; discern temperature variances through touch.

Licensures and Certifications: Active California Registered Nurse (RN) License and Basic Life Support (BLS) certification issued by the American Heart Association required. Other certifications and competencies as specified in the unit/department-specific Plan for Providing Care required. Certified Diabetes Educator preferred upon hire and required within three years from hire date.
