Job Overview
We're seeking passionate, driven advanced practice providers to join our quickly growing team. This is a part-time contract position with a per-patient-visit compensation structure equivalent to an annual salary equivalent range of $115,000 to $135,000 as a contractor, depending on your availability.
Responsibilities
- Provide comprehensive wound care to patients, including assessment, treatment, and ongoing monitoring.
- Develop and implement individualized care plans for patients with complex wounds (e.g., diabetic ulcers, pressure ulcers)
- Collaborate with Medical Director to ensure optimal patient outcomes.
- Educate patients and their families on wound care techniques and prevention strategies.
- Conduct utilization reviews to assess the appropriateness and effectiveness of wound care interventions.
- Perform triage for patients with chronic wounds, prioritizing care based on severity and urgency.
- Collect and process specimens for laboratory testing as needed.
- Stay updated with the latest in wound care research and integrate new evidence-based practices into patient care.
Skills
- Some knowledge of wound healing physiology and evidence-based wound care practices
- Proficiency in utilization review processes and documentation requirements
- Strong triage and assessment skills to effectively prioritize and establish patient care plans.
- Demonstrated ability to accurately and comprehensively document patient assessments, treatment plans, progress notes, and outcomes in the Electronic Health Record (EHR) system.
- Familiarity with specimen collection processing.
- Excellent communication and interpersonal skills
