Position Summary :
We have an exciting opportunity to join our team as a Patient Coordinator - Infusion - Full Time, Days - Infusion Department - Full-Time - Varied Day Shif.
In this role, the successful candidate As a Patient Coordinator you will be responsible for ensuring that appropriate patient treatment plans are followed Coordinates follow up care plan needs for patients by scheduling appointments or enrolling members in programs assesses member compliance with medical treatment plans via telephone or through on site visits

Duties / Responsibilities :
Minimum Qualifications :
To qualify you must have a Experience with word processing and computer databases required Strong oral and written multitasking detail oriented organizational customer service and interpersonal skills Education and Experience : High school diploma or GED required One year of office experience required; two years preferred
Preferred Qualifications :
1 2 years scheduling experience in a medical office or a healthcare setting
Qualified candidates must be able to effectively communicate with all levels of the organization.

Long Island Community Hospital provides a salary range to comply with the New York state Law on Salary Transparency in Job Advertisements. The salary range for the role is $37,584.69 - $50,212.50 Annually. Actual salaries depend on a variety of factors, including experience, specialty, education, and hospital need. The salary range or contractual rate listed does not include bonuses / incentive, differential pay or other forms of compensation or benefits.
